Merkley commented on the article referencing hospital price transparency which was a rule mandated by CMS in January 2021. Brookings Health System’s pricing can be found on our website. Between South Dakota and Minnesota hospitals, there is 81% - 100% compliance with the price transparency rule. Merkley also commented on the “Weighing a Heavy Toll” article stating the toll of the pandemic has gone well beyond caring for patients; staff are feeling overwhelmed and exhausted while also needing to care for their own children and families. Internally, Brookings Health implemented a Peer Support Team and doing other small acts of kindness to address some of the impact on our health care staff. 

 

Merkley also noted, with workforce shortages at all-time highs across the state and country and expected to rise as demand continues to grow, we are seeing an increase in temporary staffing agencies and placements being secured through such companies. Due to the challenges within our workforce, it is not uncommon to see those agencies charge facilities extremely high rates for RN’s, CNA’s and other technicians. Up to this point, BHS has utilized minimal hours of agency work and is hopeful to continue that trend and not have to pay those high rates for staff. Additionally, BHS is seeing staff beginning to break away from traditional employment through the health system and chase additional dollars they can receive through these traveling agencies.

 

Executive Reports

CEO Report

Merkley noted during the month of October we are focused on our Employee Giving Campaign encouraging all employees to support fellow co-workers, patients and our community by making a gift in any amount to either Brookings Health System Foundation or Brookings Area United Way or both. Brookings Health is also preparing for an onsite visit in November from the State Department of Health to assess the facility’s implementation of key COVID-19 Vaccination Program requirements. 

 

CFO Report

Wagner provided an update on required reporting for Provider Relief Funds received in 2020. The Health Resources & Services Administration (HRSA) recently updated the language related directly to COVID-19 costs which allowed other hospital operational expenses to be entered as use of funds. Wagner also noted an application has been submitted for Phase 4 ARP (American Rescue Plan) Rural funds available for healthcare providers.  

 

CNO Report

Hillestad shared the first surgery with our new da Vinci XI robot was a great success and surgeons are very appreciative to have the new technology. Hillestad also noted the Medical Marijuana Task Force is working on finalizing internal policies as well as procedures for drug testing and documentation for impaired employees under the influence while at work. 

 

Board Committee Reports

Administrative Committee 

Committee member Morgan provided a report; the Administrative Committee met on October 19, 2021. Topics of discussion included Meditech Expanse, Workforce, da Vinci XI Robot, Open Enrollment, Medical Marijuana, SDSU/BEDC/Vision Brookings donations, City of Brookings Scholarship Fund, FY22 Capital Budget, Board/CEO Evaluation Process, 2022 Meeting Dates and COVID/Vaccinations. 

 

Finance Committee

Committee member Anderson provided a report; the Finance Committee met on October 21, 2021. The committee reviewed and approved the payables, adjustments and financials for September. Other topics of discussion included FY22 Capital Budget and FY22 Operating Budget. Members reviewed the proposed 2022 Capital Budget; total capital requests are set at $1.73M. Of this, $260k will be funded from a state grant that is earmarked for new air handling units and a transport ventilator bringing the total to $1.47M. Contingency items identified for equipment that may need to be replaced total $86k. 

 

            A motion was brought forward from the Finance Committee recommending approval of the FY22 Capital Budget as presented. The motion was seconded by Krogh. 

 

            Action: All present voted yes; motion carried.  

 

Quality Committee

Quality Committee Chair Ellsworth provided a report; the Quality Committee met on October 19, 2021. Jaclyn Rauen, Inpatient Director, presented information on the Inpatient Wound Care process improvement project. Other topics of discussion included the Home Health survey process, Service Team Updates and COVID/Vaccinations Update.

 

Old Business

COVID/Vaccinations Update 

Merkley stated, this week, we are seeing an increase in our census due to COVID-19 illness in unvaccinated individuals and the positivity rate continues to be higher than we would like to see for the community. Some of these patients are very ill and we are struggling to transfer them to Sioux Falls due to the same workforce shortage and capacity issues happening there and across the Midwest. While we are exhausting many of our resources and staff, we are still able to take very good care of these patients. Calls continue to come in from neighboring states who are trying to place ill patients at other hospitals. At this time, we have not heard any news regarding the federal Covid vaccine mandate. Plans have been made for a POD (Point of Dispensing) COVID-19 booster event which will be held next Monday and Tuesday (November 1 and 2) at the Swiftel Center. We are still in a holding pattern on vaccines for 5-11 year olds but looking into what the demand may be within our district. 

 

New Business

Medical Staff Credentialing 

A motion was made by Ellsworth, seconded by Morgan to approve the credentialing and re-credentialing as recommended by the Medical Executive Committee. 

Action:  All present voted yes; motion carried.  

 

2022 Board Calendar

Proposed board and committee meeting dates for 2022 were reviewed. As in the past, the proposed calendar reflects meetings will not be held in the months of May, July and November. The main difference is at the committee level; with the discontinuation of the Building Committee, the Finance Committee is proposed to be moved to Tuesday following the Quality and Administrative committees.

 

A motion was made by Darnall, seconded by Morgan to approve the 2022 meeting date schedule. 

            

Action: All present voted yes; motion carried. 

 

CEO/Board Evaluations

Anderson provided information regarding the annual Board and CEO evaluations. The CEO evaluation was created to align with Brookings Health System’s mission, vision and values and the board evaluation is based on responsibilities listed in the board orientation manual.  Both use a 5-scale rating system and allow for comments, which are most helpful in understanding the rational for the score. Members were asked to complete the evaluation forms and return them to Anderson by December 1st, which will allow enough time to compile results for the December 7th Administrative Committee meeting. The Medical Staff CEO Performance evaluation will also be distributed to providers. A final report will be presented at the December Board meeting. 

 

Upon motion by Morgan, second by Anderson the meeting adjourned at 6:19 p.m. The motion carried by unanimous vote.
